At this point LS-CAT does not offer a routine mail-in service to members, general users, or industrial users. By special arrangement, however, mail-in is feasible. If you have been offered mail-in data collection time then you MUST include the following form with your shippment:

Do NOT fill out this form until you have contacted the LS-CAT staff and have arranged for someone to collect your data for you.
